Parent Agreement & Code of Conduct

Please review this agreement before registering your child for YSMnation Founders Lab. By completing registration, you acknowledge that you have read, understood, and agreed to the terms below.

Parent/Guardian Acknowledgment: This page is designed to help families understand program expectations, safety guidelines, liability terms, media permission, and Market Day responsibilities before sign-up.

Assumption of Risk

I understand that participation in any youth program may involve some risk. These risks may include, but are not limited to, minor cuts, scrapes, bruises, slips, falls, allergic reactions, illness, emotional discomfort, conflicts with other participants, or accidents involving age-appropriate materials, supplies, tools, or activities.

YSMnation will make reasonable efforts to provide a structured, supervised, and age-appropriate program environment. However, I understand that no program can eliminate all risks.

Medical Authorization

In the event of illness, injury, or emergency, I authorize YSMnation staff or representatives to contact me and/or the emergency contact listed on my child’s registration form.

If I cannot be reached promptly, I authorize YSMnation staff or representatives to seek emergency medical care for my child when reasonably necessary.

I understand that I am responsible for any medical expenses related to such care.

I agree to provide accurate information during registration regarding my child’s allergies, medical conditions, medications, behavioral needs, or any other important care instructions.

Release of Liability

To the fullest extent permitted by law, I agree to release, waive, discharge, and hold harmless YSMnation, Young Success Makers, its owners, directors, instructors, staff, volunteers, contractors, partners, host location, and representatives from claims, losses, damages, injuries, or expenses arising out of or related to my child’s participation in Founders Lab.

This release applies to claims connected with ordinary risks associated with participation in the program, except where such claims result from gross negligence, willful misconduct, intentional misconduct, or conduct that cannot legally be waived.

Personal Belongings

I understand that my child is responsible for any personal items brought to the program. YSMnation is not responsible for lost, stolen, or damaged personal belongings.

Parents are encouraged to label personal items and avoid sending valuables that are not needed for the program.

Photo and Media Permission

During registration, parents/guardians will be asked to select a media permission option.

If I give media permission, I authorize YSMnation to photograph or record my child during program activities and to use those images or videos for program highlights, website content, social media, promotional materials, recap materials, or educational purposes.

If I do not give media permission, YSMnation will make reasonable efforts not to intentionally use my child’s photo or video in public promotional materials.

I understand that group photos or event images may sometimes include participants in the background, and YSMnation will make reasonable efforts to respect the media preference selected during registration.

Market Day Participation

I understand that Founders Lab may include a Market Day, showcase, or presentation where students may display, pitch, or sell student-created products or business ideas.

I understand that students will receive guidance, but parents/guardians are responsible for reviewing and approving their child’s product idea, materials, pricing, and participation.

YSMnation may restrict or prohibit any product that is unsafe, inappropriate, food-related, not age-appropriate, or not aligned with program guidelines.

Students may not sell food, unsafe items, restricted items, or products that have not been approved by YSMnation staff and the parent/guardian.
